AEP Health Group and the Commonwealth Home Support Program
The Commonwealth Home Support Programme (CHSP) helps older Australians access entry-level support services to live independently and safely at home. If you’re having trouble with everyday tasks and feel that a little support could improve your health and wellbeing, CHSP could be right for you.
As a registered provider of CHSP, AEP Health Group can now provide allied health services that include Physiotherapy, Exercise Physiology and Dietetics. These services can be accessed at one of four clinic locations as well as in your home. Our services aim to assist you, but not limited to the following areas:
• Improve balance and strength
• Increase mobility and independence
• Weight management
• Pain/symptom reduction and management
• Increase social interaction
If you are already registered with CHSP and eligible for allied health therapy services, simply complete the online form. Our friendly admin staff will then be in touch to get you booked in.

Am I eligible for The Commonwealth Home Support Program?
The first stage is to find out if you are eligible for subsidised aged care. This involves a two-part assessment process that understands your needs and what services could help you. It starts with a simple eligibility check which you can do online or over the phone followed by an in-person assessment.
How to apply for an assessment
The first step to access government-funded aged care services is to get assessed. You can apply for an assessment online or call My Aged Care on 1800 200 422.
The online application is quick and easy using your Medicare card. If you do not have a Medicare card, or would prefer to speak to someone, you can call us to complete the application over the phone. If you would prefer to apply in person, you can make an appointment to see an Aged Care Specialist Officer (ACSO) at a number of Services Australia locations. To book an appointment with an ACSO, call the Services Australia Aged Care line on 1800 227 475. They are open Monday to Friday from 8am to 5pm.
